- Completed Notice of Application Form 59.07 Form 59.07 , with ‘registration of agreement (s. 52)’ checked off under the Parenting and Support Act.
- Completed Statement of Contact Information and Circumstances Form FD1 Form FD1.
- Completed Personal Representation Form Form if the registration request is being made by a self-represented party.
- Original, signed version of the separation agreement (original with ink signatures from both parties, dated, witnessed etc.), plus, 3 photocopies of the separation agreement .
- Filing Fee: $43.60 (as of April 2015) (fees are subject to change without notice) OR completed Waiver of Fees form with proof of your income attached.
